Submit a Claim - Claim Instructions
By timely submitting this Claim Form, you may be eligible to receive the benefit identified in the Class Notice and the Settlement Agreement and Release. If you also submit a request for exclusion (in other words, if you ask to “opt out” of the Settlement Class), this Claim Form will be deemed invalid.
You may complete and submit the Claim Form online, or you may complete the Claim Form to mail in. You may only submit one Claim Form via one method of your choosing.
Claim Forms must be submitted online no later than February 19, 2025, or postmarked (if mailed) no later than February 19, 2025. To be valid, your Claim Form must be complete, accurate, and timely. Your Claim Form must also include a completed attestation regarding the accuracy of the information submitted. Your Claim Form may be rejected by the Claim Administrator if any of these conditions are not met.
If you fail to submit your Claim Form by February 19, 2025, your Claim will be rejected, and you will be deemed to have waived all rights to receive a Monetary Benefit under the Settlement.
